Dear Quality of Life INNOVATIONS (QLI) Alumni,

To celebrate QLI’s 25th Anniversary, W!se is doing a survey to track and celebrate our alumni journeys since QLI. The program has been lucky enough to play a part in the success stories of so many you and we would love to be able to share that with our stakeholders so we can give even more students the opportunity to benefit from the QLI program. 3. Telling us if you would be interested in giving back to the next generation of QLI participants.

Everyone who takes the survey before September 15th, 2019 will be entered into a prize draw to win $50 Amazon gift certificate. We will also use this information to select QLI Alumni to win awards for their work since the program started at QLI’s 25th Anniversary Awards Ceremony.
